| straight catheterization |

today i watched a man get straight cath'd. -you stick a catheter [which looks waaay too big to fit into his piss hole] into his urethra and let the urine drain out.
usually its done after surgery for a patient who was under anesthesia, because anesthesia shuts all your GI down, making your bowels and bladder all goofy and sleepy. so when you wake up your body doesnt know you have to pee, and then you feel like crap cuz you cant tinkle. so we straight cath ya and let it all drain out on its own into a little bucket thing. its called a straight cath cuz you stick it in and take it right back out after all the urine is drained. normal caths have a balloon on the tip, that stays inside your bladder and acts as an anchor so it doesnt slide out. catheters should only stay in for a max of like 5 days because bacteria get up in there easily and give you a UTI.

 look familiar?
this is a portrait of Dr. Gross, an American academic trauma surgeon lecturing a group of Jefferson Medical College students. It was painted by Thomas Eakins in 1875. "Admired for its uncompromising realism, The Gross Clinic has an important place documenting the history of medicine—both because it honors the emergence of surgery as a healing profession (previously, surgery was associated primarily with amputation), and because it shows us what the surgical theater looked like in the nineteenth century. The painting is based on a surgery witnessed by Eakins, in which Gross treated a young man for osteomyelitis of the femur. Gross is pictured here performing a conservative operation as opposed to an amputation (which is how the patient would normally have been treated in previous decades). Here, surgeons crowd around the anesthetized patient in their frock coats. This is just prior to the adoption of a hygienic surgical environment (see asepsis). The Gross Clinic is thus often contrasted with Eakins's later painting The Agnew Clinic (1889), which depicts a cleaner, brighter, surgical theater. In comparing the two, we see the advancement in our understanding of the prevention of infection."
the original painting used to hang in the hall in one of my buildings, but in 2006 was sold for $68 million to the National Gallery of Art in Washington. today after clinical we went over to Alumni Hall to visit the little museum that houses 3 replicas of Eakins' work. it also has a ton of creepy horror movie-looking surgical tools from Gross' time. |

| | sweet- teenage rampage | ] | today i taught my patient how to take care of a colostomy. He just had a temporary one placed, and will keep it for about 8 weeks. my instructor and i changed his abdominal wound dressings and then coerced him into letting us change his colostomy bag and wafer before i had to leave for the day. we even had him participate. my instructors were so proud of me. his poo didnt smell nearly as foul as my first encounter with a stoma. hahaha! much better for me and the poor guy.
before all that, while my patient had visitors, i helped another nurse with her patient. the guy was super sweet. I took off his old dressings, which were "packed" and "wet to dry" ones. basically, we soak gauze in saline water, then stuff it into the gaping whole in his gut. so i did that, and he had a colostomy too. those things are super popular on my floor. i got to put my name on his dressing. hehe. i was busy from 7am until almost 2pm, thats the longest day i've had at the hospital so far. busy busy.

| | tired | ] | my patient last week was good. i finally got a competent alert person! i went down with her to have a CVIR drain placed. she'd had a small bowel resection and an abscess formed in her abdomen and needed to be drained. we all wore xray shields and cute little head caps. the procedure was quick, maybe an hour. the 2 Dr.s used an Xray machine to do the procedure, only needing to make one 1inch incision, then using a wire to place the drain. i was standing 5 feet from my patient the whole time. it was awesome. they let me see everything and explained stuff to me. once the drain was placed, the withdrew 4, 20mL syringes of brown-yellow pus. it was so cool. the poor patient. after the procedure, she loved me even more because i took such good care of her. <3 <3 |

its silly that on the actual holiday i didnt do anything crazy. i did sit around with Julie and her friend from Cali and watch old music videos. and came up with some more party themes. -peanuts are forbidden from all future parties.
midterms are in 2 days. gotta get down to business tomorrow!!! |

| | dixie chicks | ] | i had my first Flu Clinic yesterday! it was awesome. i love stabbing people with needles!! and im so proud of myself, my instructor made everyone check out my kick ass technique. i've got some steady hands. My clinical group went to 2 churches to give the shots. and a ton of old people showed up. i didnt really do anything difficult, but i was exhausted after everything. maybe it was the dreary rain and long car rides. actually, i loved the weather, it must have been b/c the car ride. after i got home, i hopped on the train to see Alex so she could do a makeup test run. she made me look redonk/awesome. im so pumped for my party!
I was at the hospital today. my patient today was almost 80 and a complete rag doll. she spurted jibberish the entire day, and was limp as a sac of potatoes. i watched her nurse change her abdominal wound dressing. that shit was deep and open. freaky. and i helped change her colostomy bag. aka her SHIT BAG. it stank pretty foul. but seeing her ostomy was awesome and gross. her wound looked a little like this. the chick in this pic is holding a colostomy bag.
 when my patient passed out, i went to help another student make an obese man's bed while he was in it. poor guy had hip surgery and we kept hurting him. he was super sweet.
last week my patient had MRSA. i had to put on a gown before going into his room. he was pretty chill though. he had a bunch of bandages on his abdomen and a J tube, which drains bloody fluid from the wound. he also had a feeding tube going right into his gut. i, along with an extern nurse, gave him a bed bath. my first one in a hospital! i saw some old balls. but nbd, i see genitals basically everyday. old ones. |

| | Eyes, Lips, Body remix- Ramshackle | ] | I had my second patient today! she had a partial L nephrectomy because of a tumor. it has been growing for the past 4 years but it caused no symptoms so she just let it go. crazy. She was old and super sweet and cute. and i didnt have to give her a bath. I get all the independent patients.
I have 3 exams next week. a ton of reading to do. i'm finished my paper, but I just realized I have another project due on Tuesday. I think thats it though.. I hope!
Julie's mom leaves in the morning :( no more yummy asian foods. I like having an extra someone living here. Im so used to living with a million people. I miss having 10 people in my house.. for the most part.
btw i forgot to mention, i emptied one of these! its a JP drain. it drains fluid from a wound! my patient had 20cc when i emptied it. hhehehee yayy

I would have liked to have been more hands on with the care. Oh well, theres always the next 2 years for that. The staff is cool. nice and helpful. and all young girls. and there are so many fine ass Drs walking the halls, its great. oh and there was a patient on my floor with MULTIPLE drug resistant organisms... creeepy!

human immunodeficiency virus destroys your immune system, and at the chronic stage becomes acquired immunodeficiency syndrome. your immune system can't fight infection, so many patients die from illnesses that a healthy person would fight off without medication. Medications are used to help the body fight pathogens, so if its depressed, no amount of meds will help, because theres no one in there to make stronger. mothers with hiv/aids are given antivirals during pregnancy to prevent transmission of the disease to offspring. WOO science.
patients are not obligated to omit infection by this disease....so we have to pretend that everyone has aids. scary. |

| | optimistic | ] | I have been in school for 2 weeks now. i've learned so many wonderful, interesting things. mondays and tuesdays are lecture days. ive had 2 medcal tests, both of which i got 100% on. after that i have my Intro to Pro Nursing class from 2-5pm. so far we've talked about nursing's history, Florence Nightingale and how she was the first nurse. she introduced sanitation to healthcare facilities and developed the Pie Chart. you go girl. tuesdays are medcal review, which i skip, and Computers in Nursing. which is a class for all the older career-changers in our class that dont know how to use Word and Excel. kind of a waste of time, but its an easy A. Wed, Thurs, and Fri are lab days. 8-10am lectures, then 1:30-4pm clinical labs where we practice what we've learned. so far i have checked vitals on my classmates, inserted a catheter into a female mannequin, inserted a nasogastric tube, learned how to bathe a patient in a bed, how to change patient's positioning, how to get a patient into a wheelchair, change bedsheets, learned how to wash my hands, how to perform sterile technique, and today, we are learning about bowel elimination and stomas. Stomas are fucking repulsive. i can't wait to see one in real life!! theyre red, juicy little monsters that spurt poo. It is literally your intestine sticking out of your abdomen. gnarly.

| | tired | ] | the bar across the street from us has $1 drinks every Thurs night for jeff students. we will be there every week. friday Julie and I met up with Ferry and some people at some hipster bar in the middle of nowhere in north philly. they had super tasty drinks and LOUD music. hipsters are everywhere in this city. ugh. every boy is adorable and smelly. i love my rooms. she is fun and nice and loves drinking and dancing as much as i do. shes exactly what i wished for. and she also met an adorable gay boy to be her bff. so now the 3 of us play together. saturday sam came down to see zach's play, but before that she met up with me and jul and we went to South Street. i gave them a mini tour and we went shoe shopping. I finally made it to T Mom. food was alright but overpriced. ill need to try more drinks to see what all the hypes about. julie and i went to a show for about 10 mins. music was good. we came back to the apt and drank wine and watched Frida. such a long movie. and SH has some niice boobs. bed around 11.
at 2:30am, i was awoken by a blaring siren wailing in my ears. the fire alarm had been set off and the entire building was evacuated. we were only outside for 20 mins before they let us back in. everyone was pissed. I overheard a girl say that this happens at least ever month. fuck that. i was wide awake after that, so i read some more qc and passed out around 4. then to my amazement i was once again awoken by the eery siren screaming at me to GTFO. it is so creepy to wake up to. i passed right out this time around.
i set my alarm for noon. didnt want to be sleeping in later than that. although it wouldnt have made any difference, julie and i didnt do shit today, other than cook breakfast and dinner. we watched netflix and snacked all day.
class M-F this week. home tues night for a visit. cpr class wed. possible gym adventures thurs/fri w J. |

| | snow baby | ] | today i learned how to take blood pressure using the two step method, read the level of oxygen in your blood, record respirations, and record heart beats per minute. yeehaw!
we also spent lots of time talking about washing our hands. did you know that 78,000-90,000 patients die each year from Hospital Acquired Infections? that means that the patient got an infection/illness while in the hospital, because someone didnt sanitize properly! |
